Austin Health Palliative Care

AUSTIN HEALTH PALLIATIVE CARE SERVICES

Austin Health Palliative Care Services provide comprehensive interdisciplinary assessment and care for patients with terminal illness. The Palliative Care Services comprise an Acute Consultancy Service based at Austin campus, a Palliative Care Unit which provides bed based palliative care and is situated on the Heidelberg Repatriation Hospital site and a Palliative Care Volunteer Program which provides volunteer services to patients within Austin Health facilities.  The three components of Palliative Care Services work collaboratively and link directly to community based palliative care organisations who provide home based care in the north east region.  Both the Acute Consultancy Service and the Palliative Care Unit provide holistic pain and symptom management, facilities psycho-social and spiritual support and assists in facilitating discharge planning and restorative care.

Banksia Palliative Care Services

Banksia Palliative Care provides a program of care enabling a person to live in dignity, peace and comfort throughout the late stage of their illness. Fundamental to this is the control of distressing symptoms. We have an agreement with the Royal District Nursing Service to ensure 24 hour nursing support is available. Our philosophy is to provide holistic care of people with life threatening illnesses by responding to their physical, psychological, emotional, social, spiritual and cultural needs. We recognise that patients have a right to control the service they receive. We are also responsive to the needs of the patient’s family and care givers and strive to meet cultural needs of those involved.

Melbourne Citymission Palliative Care

Established in 1854, Melbourne Citymission is an independent nondenominational Christian welfare agency, operating in Melbourne’s Northern and Western suburbs. Our Palliative Care service was established in 1981 as the first community-based service to operate in metropolitan Melbourne. We provide palliative care to people living in the cities of Hume, Moreland, Darebin and Yarra.

At Melbourne Citymission our team aims to recognise and respond to the physical, emotional, spiritual, social and cultural needs of clients and their carers. We aim to promote the dignity and independence of clients and carers through:
• facilitating care which is directed by clients themselves
• supporting clients and their carers at home
• enabling clients with changing needs to make choices regarding where they are cared for, at home, in hospital or in a palliative care unit.

Mercy Palliative Care

Mercy Palliative Care provides care for people with a terminal illness and supports the families within their own homes. Mercy Palliative Care is a community resource covering the City of Brimbank, City of Hobson's Bay, City of Maribyrnong, City of Melbourne, City of Moonee Valley, City of Wyndham and the Shire of Melton.  Mercy Palliative Care is an alliance formed between Mercy Health and Royal District Nursing Service. Our dedicated team includes: specialist palliative care physician, nurses, counsellors, family support volunteers, music therapist, pastoral care, and bereavement support. The service is free of charge and available 24 hours per day 7 days per week.

Royal District Nursing Service (RDNS)

At Royal District Nursing Service (RDNS) we believe one of the best places to receive healthcare and remain independent is in the comfort and safety of one's own home or other places of choosing.

For this reason we provide a 24 hour home health and home care service, every day of the year.  Each day, RDNS staff provide professional health and homecare to more then 9,500 people throughout Greater Melbourne, parts of regional Victoria, New South Wales and New Zealand.

We provide nursing care in the following areas: aged care, diabetes service, cystic fibrosis, HIV/AIDS support service, hemophilia care, continence management, wound care, stomal therapy, palliative care and bereavement support and post acute care.   Healthcare is also available through our Homeless Persons Program to clients experiencing homelessness.

In addition to these services, RDNS provides home care services, mainly in New South Wales, including domestic assistance, personal care, general services, monitoring, overnight services, respite care, transport and more.

Our clients' needs are RDNS' primary concern.  To ensure that all total healthcare needs are met, we will assess each situation individually and prepare a personal, tailored healthcare plan.  Where possible the involvement of family, friends, carers and other service providers will occur in planning and delivering a client's healthcare.

The Northern Health Integrated Palliative Care Service

The Northern Health Integrated Palliative Care Service provides a specialist service to improve the quality of life for persons with progressive life limiting malignant or non-malignant illnesses, as well as their families or carers.  This includes a specialist inpatient palliative care unit for symptom control, respite and end of life care at Broadmeadows Health Service (BHS), and a consult service to inpatients and outpatients at The Northern Hospital (TNH) and inpatients at Bundoora Extended Care Centre (BECC).  In addition there are weekly palliative care outpatient clinics at The Northern Hospital and Craigieburn Health Service.

The Service offers pain and symptom management, social, psychological, spiritual and emotional support including bereavement support and conselling.  We aim to work closely with hospital and community healthcare providers including GPs and other community and inpatient palliative care services to ensure seamless care.  There is a 24 hour a day, 7 day a week on call service to provide advice for healthcare professionals involved in the care of our patients.  We are also involved in education within Northern Health and in the wider community, including via the PEPA Program.

The Royal Melbourne Health Palliative Care Service

The Royal Melbourne Hospital Palliative Care service provides care to patients in three settings:

  1. Care within our dedicated 10-bed Palliative Care Unit, on ward 6 South East at The Royal Melbourne Hospital's City Campus
  2. A hospital wide consultative and referral service, seeing hospital patients and clinic patients, both at The Royal Melbourne Hospital's City and Royal Park Campus
  3. A weekly outpatient clinic, based at The Royal Melbourne Hospital's Royal Park Campus.  This clinic is a bulk-billing medicare clinic.

The service is provided by a specialist medical, nursing and allied health staff.  We also provide education to hospital and community services.  We participate in national research projects, including clinical trials and our service is affiliated with the Department of Medicine, University of Melbourne.

Referrals to the Palliative Care Unit should be made to, Nurse Unit Manager, Palliative Care Ward, on 9342 7000.

Referrals to the Outpatient Clinic, require a written referral from a medical practitioner to be faxed 9342 4928.

Werribee Mercy Hospital

The Gabrielle Jennings Centre for Palliative Care is a 12 bed inpatient unit providing care for patients across the Western Suburbs of Melbourne.  We work closely with Mercy Palliative Care, the community based Palliative Care team.

The Gabrielle Jennings Centre is a purpose built 12 bed unit comprising eight single rooms and  two 2 bed rooms.   Care is provided for patients requiring symptom control, terminal care and respite when beds are available.  There are lounge and kitchen facilities for use by patients and their visitors.  Relatives are able to stay overnight if they desire.

We are staffed by experienced Palliative Care nurses and two part time Palliative Care Physicians.   There is also a full time Palliative registrar working in conjunction with a multidisciplinary team.

Western Health

The Western Health Palliative Care team aims to provide services to Footscray, Sunshine and Williamstown campuses. The Consultancy Service crosses all campuses and provides palliative care expertise to Western Health inpatients, and liaises closely with community agencies to ensure smooth transition of care between home and inpatient settings.  This collaboration has been strengthened by the introduction of the consultancy outreach service. The inpatient and day hospice units are located at Sunshine Hospital. The 10-bed inpatient Unit provides end of life care and symptom management to palliative patients of the Western region. The Mercy and Western Day Hospice is also located at Sunshine Hospital and operates on Tuesday and Thursday.  The day hospice aims to provide day care support to patients from the Western region – this is a collaborative service with Mercy Western Palliative Care.
