Referral

Referrals to Austin Health, St Vincent's Palliative Care Service, Werribee Mercy Hospital and Western Health Sunshine require completion of the Service Coordination Tool Templates (SCTT).  The SCTT is a suite of templates developed to facilitate and support service coordination, and support the collection and recording of initial contact, initial needs identification, referral and coordinated care planning information in a standardised way.  The SCTT can be accessed at this link in both word and pdf format.  A user guide is also available at this site.

Whilst the tools in the SCTT are available for use, all health care professionals are now mandated to use the Palliative Care Supplementary Information Tool which is also available at the same site.

 

Banksia Palliative Care Services accepts referrals by contacting the Intake Worker on 9455 0822 or 9456 9744.  Health professionals are required to refer using the SCTT.

Broadmeadows Health Service accepts referrals by telephoning 8345 5196 or faxing 8345 5353.

Melbourne Citymission Palliative Care accepts referrals by telephoning 9486 2666.  Health professionals are required to refer using the SCTT.

Mercy Palliative Care accepts referrals by faxing the Intake Coordinator on 9364 9777.

The Royal Melbourne Hospital accepts non-urgent admissions by faxing 9342 4928 and urgent admissions ringing 9342 7000.
