Clinical Reviews / Needs Rounds

Palliative Care Clinical Reviews / Needs Rounds are monthly 60-minute staff triage meetings, where aged care staff and palliative care clinicians review and discuss the status of 4-6 residents in the aged care facility. The idea is to focus on those who are at greatest risk of dying without a plan in place and who have a high symptom burden.

A valuable adjunct to Clinical Reviews is death audits. Click here to access the latest version of the ELDAC death audit tool.
